A Few-Millimeter Brain Difference in MRI Scans of 39 Men Diagnosed as Psychopaths That No Clinical Interview Could Detect

Studies conducted in Germany have provided physical and measurable evidence of structural anomalies associated with psychopathy, relying notably on magnetic resonance imaging (MRI). For the researchers, their results bring new insights to the topic of violent behaviors.

A study of 39 men with psychopathy

In general, psychopathy is a neurodevelopmental personality disorder characterized by a striking lack of empathy, emotional detachment, a propensity for manipulation, and impulsive, antisocial or even violent behaviors. Very present in cinema and literature, the notion of a “psychopath brain” is not a measurable reality. At least, that was the case until recently.

The Jülich Research Centre (Germany) published a study on the subject in the journal European Archives of Psychiatry and Clinical Neuroscience in May 2025. The researchers analyzed data and carried out a microscopic brain anatomy mapping of 39 men with psychopathy, using MRI and the three-dimensional Julich-Brain brain atlas.

An antisocial and violent tendency that leaves physical traces

Concretely, the researchers’ analysis involved the Hare Psychopathy Checklist-Revised (PCL-R), the global reference tool in the field. This scale separates the disorder into two fundamental dimensions: Factor 1 involving emotional coldness, lack of empathy and superficial charm (interpersonal/affective traits), and Factor 2 relating to instability, delinquency and the inability to control impulses (violent behaviors). According to the results, there is notably a physical reduction of gray matter. The study authors noted especially high scores for Factor 2, which were strictly correlated with a significant reduction in the volume of several key brain regions.

More precisely, the affected areas are the thalamus, the basal ganglia, the cerebellum, the brainstem pons, the insular cortex and the orbitofrontal cortex. These latter—major subcortical and cortical structures—form the network responsible for managing emotions, interpreting stimuli, and regulating actions. In plain terms: the brains of violent profiles display a “wiring” with substantial gaps in the execution of impulse control.

The German study thus shows that emotional coldness (Factor 1) leaves few physical traces, which is not the case for the antisocial and violent tendency (Factor 2). Indeed, the latter is deeply anchored in the brain’s biological structure.

A pathology of neural communication networks

A second study (meta-analysis) published in Neuroscience & Biobehavioral Reviews in September 2025 and led by the University of Birmingham (United Kingdom), echoes the first. It targeted the greatest challenge in the neuroimaging of psychopathy: the replication deficit. Undoubtedly, this study represents a major work concerning the notion of the “psychopathy network”. For the British researchers, this psychopathy network is primarily composed of regions of the default mode network and the subcortex.

Rather than attempting to target a single isolated area of psychopathy, the scientists used the functional connectome from 1,000 healthy participants to test another hypothesis. They discovered that these disparate localizations actually belonged to a single interconnected network, with a replication rate of up to 87.5%.

Thus, together the two studies confirm that modern research has finally moved beyond the simplistic view of the “psychopath brain,” often depicted in cinema. Indeed, it is now a matter of a true pathology of neural communication networks.
